14th April - Introduction to dermatoscopy and my approach to lesion diagnosis

  • An in-depth rethink of how I believe we should approach lesion diagnosis, with a focus on the key features in the history and examination of lesions. Why relying on the ABCD method can result in missed melanomas and unnecessary urgent referrals for benign lesions.

 

21st April - What dermatoscopy is all about, choosing a dermatoscopy and tips of taking a photo

  • Guidance on choosing and using a dermatoscope, the differences between polarising and contact modes. Tips on taking an image  (including consent, common pitfalls and making effective A&G referrals).

 

28th April - Dermatoscopy of the obviously benign lesions

  • The dermatoscopic features of Seb K, dermatofibroma and haemangioma (and more)

 

19th May - Pigment network patterns and vessels 

  • An overview of the six main pigment network patterns (PNPs), the importance of a patient’s mole ‘signature pattern’ and the various vascular patterns that can help clinch the diagnosis.

 

2nd June - Basal Cell Carcinoma

  • The macroscopic and detailed dermatoscopic features of superficial, nodular, micronodular and morphoeic BCCs, including the pigment and vessel patterns.

 

9th June - Signs of melanoma part 1

  • The 20+ dermatoscopic clues to melanoma including abnormal PNPs, the negative network pattern, and pink and brown lesions

 

30th June - Signs of melanoma part 2

  • Continuing the theme and covering bright white lines, regression, veils, chaos and more.

 

7th July - Flat pigmented facial lesions

  • A review of the dermatoscopic features of solar lentigos, Seb Ks and lichenoid keratoses on the face, and the features that help to distinguish these and pigmented AK from lentigo melanoma.

 

8th Sept - Acral lesions, nails unit melanoma and pigmented mucosal lesions 

  • A detailed analysis of acral skin, including verrucous carcinoma and  acral lentiginous melanoma. A discussion on the few ‘rules’ regarding nails and pigment, distinguishing benign melanonychia from melanoma, and the other features of nail unit melanoma. Brief discussion regarding mucosal melanoma and melanosis

 

15th Sept - The keratotic series

  • My thoughts on actinic keratoses and their management. The dermatoscopic  features, diagnosis and management of AKs, porokeratoses, Bowen’s disease, keratoacanthomas and SCCs. Although less useful in the presence of a lot of keratin, dermatoscopy still has a valuable role in such cases.

 

Biography of Speaker:

Dr George Moncrieff has been a GP in the U.K. for over 40 years.

In 1999 he became one of the first GPwSI in Dermatology and ran a highly successful Interface Dermatology Service in the community receiving direct referrals from around 50 local GPs.

In the past he’s been an external examiner for the Cardiff Diploma in Practical Dermatology, a senior examiner for the Diploma in Child Health for The Royal College of Physicians, a GP trainer and an undergraduate tutor in Medicine at Oxford University and the Royal Free Hospital in London. He was the RCGP Dermatology Curriculum Guardian from 2010 until 2014, Chair of The Dermatology Council for England from 2014 until July 2018 and a PCDS Committee Member from 2005 until 2018. He remains a Primary Care Advisor to the National Eczema Society.

After retiring from his NHS Practice in 2017, he worked in a private practice in Oxford, as well as doing locums for local GP practices.

In 2019 he helped to set up a Community Dermatology Clinic in South Birmingham, where he still remains involved and is consulted on an almost daily basis for advice and support by the GPwERs who run it.

Currently, his main role is to provide dermatology teaching in Primary Care, as well as occasionally to dermatologists and plastic surgeons.
