The Care Choices Service (formerly the Compassionate Companion Service) is expanding to support more people who are identified as being in their last year or so of life, including those living with severe frailty who are not always picked up early enough by GPs, hospitals or community teams, often resulting in crisis management, increased INT workload and moral distress.


As part of the national neighbourhood model and the shared ambition to care for more patients well in the community, the ICB is asking us to scale up the service by working more closely with community partners. This training will offer practical updates and upskilling in frailty, and show clearly how the Care Choices Service can work alongside Integrated Neighbourhood Teams to identify and support appropriate patients, helping to reduce crisis work and emotional burden rather than creating extra tasks or responsibilities.
