Overview:

Many people have multiple long term conditions, which are currently reviewed separately by general practices. This seminar will demonstrate how practices can conduct integrated, personalised reviews of all the patient’s conditions together. By focusing on each individual’s real needs, this is better for both patients and practices.

Learning Outcomes:

  • Learn about the scale of the problem.
  • Hear our nine-step plan for how to introduce personalised long term condition reviews.
  • See specific tools we have developed to help.
  • Learn about the evidence of benefit and common issues that general practices and ICBs need to address in order to make this change successfully.
