Would you like to organise Learning Disability Annual Health Checks more efficiently for your team and your patients?

Do you want annual health checks and health action plans to feel more meaningful, and to understand how the Learning Disability & Autism (LD&A) Liaison Team can support you and the people you care for?

To help improve outcomes, the SNEE ICB is hosting 2 Lunch and Learn sessions.

These sessions will bring together:

• Two GP practices from the Ipswich and East Suffolk Alliance
• The NSFT Learning Disability & Autism Liaison Team
• ACE Anglia, including people with lived experience

Focus: Sharing best practice across GP practices

• Stow Health (LD-friendly GP Practice) will share how they deliver the annual health check process from an administrative perspective
• Two Rivers will outline their clinical approach.
• Each practice will describe their pathways and processes, with David Cargill facilitating discussion and prompting further detail.

The sessions will explore what good-quality annual health checks and Health action plans look like in practice, and how working together can improve support for people with a learning disability.
